- 1 May 2012 for documentaries completed between 1 August 2011 and 1 April 2012.
- 1 August 2012 for documentaries completed after 1 April 2012.

In order to access the form and submit a film, you need a MyIDFA account. This will allow you to submit films and projects to IDFA as well as to the Docs for Sale and FORUM markets.
